What is the V.I.P. Referral Program?
We are asking you to tell your friends about Madison Family Dental. In return, you’ll receive more than a pat-on-the-back... get REWARDS in the way of special goods and gift certificates to area businesses.
How Do I Refer Friends?
Simply give your friend a “V.I.P. Referral Card” and print your name in the “My Friend” blank. The friend must then submit the card at their first appointment to the hygienist or receptionist. You will be notified via telephone when you are eligible for your reward.
WHAT ARE THE V.I.P. Referral Program Rules?
Eligibility – Referer: An “eligible referrer” does need to be a current MFDA patient to receive a gift under the Program.
Eligibility – Refered: Eligible referred individuals” under the Program cannot be current or former MFDA customers.
Referal Process: An “eligible referrer” identifies an “eligible referred individual” by completing and submitting a V.I.P. Referral Card
from MFDA. No other form of submission will be accepted. Eligibility is at the sole discretion of MFDA.
Right to Modify: MFDA reser ves the right to change or discontinue the V.I.P. Referral Program at any time without notice.
REWARDS Process: Referrer will be contacted via phone regarding earned Rewards products and gift certificates. Referrer must pick up Rewards during regular business hours within 30 days of notification. After 30 days, Reward is subject to forfeiture.
Entire Agrement: These Rules and Regulations represent the entire agreement between MFDA customers and MFDA with respect to the V.I.P. Referral Program.
